I-140 is the employer's or self-petitioner's petition. It is filed on its own when no visa number is available, and concurrently with the I-485 when one is.

I-485 — Application to Register Permanent Residence or Adjust Status — Filed concurrently when a visa number is available and the worker is already in the U.S.
I-765 — Application for Employment Authorization — Work authorisation for the worker, filed with the concurrent I-485 rather than with the I-140.
I-131 — Application for Travel Documents, Parole Documents, and Arrival/Departure Records — Advance parole for the worker, filed with the concurrent I-485.
G-28 — Notice of Entry of Appearance as Attorney or Accredited Representative — If an attorney is appearing on the petition, as employer filings commonly do.

Is the I-140 filing fee refundable if USCIS rejects or denies my form?

These are two different outcomes. If USCIS rejects your filing, the case was never accepted — USCIS returns the package and the filing fee, but you lose the months you already waited and have to start again. If USCIS denies your case, it was accepted and adjudicated, and the filing fee is not refunded. Filing it correctly the first time is what protects both the fee and the timeline.
